Description

Mental Health Navigator – Kent & Medway

Shaw Trust is proud to be a strategic partner in Mental Health Together (MHT) – a transformed, multi-agency service designed to make mental health support simpler, more joined-up, and person-centred. Our goal is to ensure people receive the right care without navigating complex service boundaries or repeating their story.

As a Mental Health Navigator, you’ll play a vital role in bridging the gap between primary care (GPs) and community mental health services. You’ll be one of the first points of contact for people accessing MHT, ensuring their experience is positive, compassionate, and tailored to their needs.

What you’ll do:

  • Provide holistic, person-centred support
  • Deliver trauma-informed, needs-led care
  • Remove barriers to access and improve collaboration across services
  • Involve service users and carers as equal partners
  • Champion co-production in service design and delivery

About you:

  • Significant experience in community mental health or wellbeing settings
  • Skilled in supporting individuals with complex mental health needs
  • Strong understanding of local services and social factors affecting mental health
  • Excellent communication and teamwork skills
  • Passionate about improving people’s experience of mental health services
